The Alpha Dog Pet Waste Management "New Customer Information WELCOME Form" consist of two parts:
1) New Customer Information (In which you can select to complete the rest of the form over the phone instead)
2) Things we need to know in order to give you the best service.
This form will take 2-5 minutes to complete, depending on how much information you need to share with us about your family pet, and any special instructions the technician will need to access and service your yard safely.
The information you provide in this form will be kept secure in your customer file, and any information provided will be used only for the express purpose of providing our best service. Remember, you can always elect to complete part two over the phone, just let us know in question number three.
